Application for Entrance
Students must complete this application to be considered for admission to the St. Gianna Catholic Health Academy program.
The application may be completed by:
1. By printing one of the 'printer friendly' versions (links are below) of the application and turning completed application into school Counselor.
OR
2. By completing the application form electronically and emailing it to dclarke@stgianna.org in the format of a Microsoft Word Document attachment.
A faculty committee will review all applicants and their responses. Following the review of the applications, select students will be contacted for a follow up interview to complete the application process.
